Dr. Sandeep Nayak shares updates on latest breast cancer surgery and procedures

There are multiple reasons why surgery is recommended for patients diagnosed with breast cancer. Oncologists or cancer specialists recommend surgeries to detect the spread of cancer, remove the tumour or cancer-affected organs/parts and relieve the symptoms of cancer, which can greatly debilitate the patients. These surgeries focus on removing the tumor, which prevents cancer from spreading to other parts of the body.

Dr. Sandeep Nayak, a leading oncosurgeon and founder of the MACS Clinic, an exclusive centre for cancer treatment in Bangalore states, ‘As oncologists, we recommend surgery to women with breast cancer as they can have a positive impact on their long-term quality of life. We explain in depth the suitable surgical options and try our level best to convince them of the surgical outcomes.

The surgical removal of the affected breasts, chest muscles and lymph nodes in the armpit is called a radical mastectomy. Conventional Mastectomy is selectively performed today, considering the availability of highly advanced and less traumatic surgical procedures, which are more effective in breast cancer treatment.

Furthermore, restoring the breast’s shape and size through reconstruction surgery is also possible. However, surgery to treat breast cancer depends upon several factors, such as the patient’s overall health, stage of breast cancer and other relevant factors.

As one of the leading oncologists, Dr. Sandeep Nayak specializes in minimally invasive surgeries through laparoscopy and robotic surgeries, which are highly precise and help the patient to recover faster, considering there is minimal trauma and blood loss. He explains in detail the latest breast cancer surgeries, including breast conservation surgeries, which have helped many patients recover. Highlighting the emergence and preference for Minimally Invasive Breast Surgery (MIBS), which focuses on making small incisions in the inconspicuous or less visible locations within the chest area to remove the tumour, the affected tissue and/or also enable breast reconstruction through these very incisions.

Breast Conservation Surgery or lumpectomy with Oncoplastic surgery is a minimally invasive surgery. In addition, use of intraoperative radiotherapy (IORT) reduces the treatment time making it very comfortable for the patient. They also remove some of the normal breast tissue around it and if required, the lymph nodes under the arm for further testing related to the spread of cancer.

The oncoplastic surgery involves shaping the breast after removing the cancer to give it an everyday look and feel. If required, the surgeon will perform local tissue rotation or transfer tissues from other locations in the body or use silicone pads to give a normal appearance to the breast.

If required and agreed upon, breast reconstruction surgery can be performed during this procedure, thus removing the need or undergoing another surgery. Minimally invasive surgery ensures better surgical outcomes regarding removing the tumour and treating cancer.

He also mentions the Skin Sparing Mastectomy and Reconstruction, performed in large-sized breast cancers or cancer detected in multiple locations in the same breast for which the entire breast tissue is removed.

The surgeon performs breast reconstruction through tissue transfers using the same incisions that remove the cancer-affected tissue. Depending on the patient’s parameters, the surgeon will opt for a TRAM, LD, or DIEP flap for breast reconstruction, while the nipple is reconstructed through the tattoo technique.

Similarly, the Nipple Areola Skin Sparing Mastectomy and Reconstruction surgery, which involves the removal of the entire breast tissue without removing the nipple, is performed through one or two small incisions over the breast. These very incisions are used to reconstruct the breast using tissue flaps, giving it a normal shape and contour.
